SAMUEL FORREST. Born in Conway, N. H. Age 23 years. Enlisted December 9, 1863, in Company H, 2d Regiment United States Sharp Shooters. Mustered into United States service December 12, 1863. Died of disease August 29, 1864, and buried in Cypress Hill Cemetery, Long Island, New York, in grave No. 941. Headboard marked with his name, regiment, company and date of death. United States bounty, $300; from town of St. Johnsbury, $300.
